Ongoing Support Beyond Age 25

Support Does Not Have to Stop at 25

We understand that some care-experienced young people may need support beyond the age of 25, particularly if they have a disability, learning difficulty, physical or mental health needs, or other vulnerabilities.

As you approach 25, we will talk with you about the support you may need in the future and help make sure you are connected to the right people and services so that you continue to receive the help you need.

Support From Adult Social Care

If you have a disability, learning difficulty, autism, additional needs, or require ongoing support with daily living, we can help make a referral to Adult Social Care.

Adult Social Care can complete a Care Act Assessment to look at your needs and identify what support you may be entitled to as an adult.

We will explain the process, help you understand your options, and support you to attend appointments if needed.

Working Together for a Smooth Transition

If you already have an Adult Social Care Worker, health professional, support worker or another service involved in your life, we will work with them to make sure there is a smooth transition and that important information is shared appropriately.

We want to make sure you do not feel passed from one service to another without support.

Helping You Access the Right Services

We can help you connect with:

  • Adult Social Care.
  • Health and mental health services.
  • Disability support services.
  • Housing and accommodation services.
  • Employment and training support.
  • Community groups and local activities.
  • Voluntary and charitable organisations.

Our aim is to make sure you know where to go for help and who to contact when you need support.

Staying Independent

We want you to feel confident, safe and supported as you move into adulthood.

Even where the Next Steps Leaving Care Service is unable to provide ongoing support after age 25, we will help you understand what support is available, make referrals where needed, and ensure you are connected to services that can help you continue to live independently, achieve your goals and have a positive future.
